 Crystal Care Center is locally owned and operated by health professionals with over 25 years of experience.

This experience gives the facility the insight to understand the needs of the residents and affords them the best possible care.
    Jerry Smith, formed Crystal Care Centers in 1992.  Jerry has been a licensed Nursing Home Administrator since 1979. During his tenure, he has served as Administrator, Consultant, and Regional V.P. with major national companies.
    After becoming disenchanted with the philosophy of corporate management, Jerry made the decision to form his own company where he would put the residents first. 
Crystal Care Center of Mansfield was purchased in January 1993, followed by Crystal Care Center of Portsmouth in March of 1993 and Crystal Care Center of Ashland in 2002.  His wife Susan, daughters Kyann, Mindy and Jennifer along with "Butch"& Tony Clark, Kim Nye and Kathy Kolesar are all active shareholders.

Corporate Compliance

An effective compliance program should provide employees and patients of Crystal Care Centers with a means for reporting violations at any time and without fear of repercussions or retaliation.

Crystal Care Centers has established a drop box system, which is available to use by any employee to report actual or potential compliance violations including those involving billing and claims submission fraud and abuse laws and regulations. Telephone calls can also be made directly to the corporate compliance officer at Pat Fisher (419-747-2666) during normal working hours.

Reports can only be accessed by the compliance officer or designee. All reports made will be investigated in a prompt and reasonable manner by the compliance officer or, if appropriate, by a member of the Compliance Committee. Individuals shall not be subject to retaliation on the part of any person affiliated with Crystal Care Centers based on reports that are submitted in good faith. Any such retaliation is a violation of the compliance program and should be reported immediately to the compliance officer.
